Creatine Monohydrate: The Most-Studied Supplement in Sports Nutrition, and What It Does for Men Who Train

Train hard six days a week and recover enough to do it again. That is the whole job. Not the hero session — the sixth one, on Saturday, when the week has already taken what it wanted.

Creatine is the single best-evidenced nutritional tool for that job, and it isn't close. Here is what it does, why the research behind it stands apart from everything else on the shelf, and how to actually run it.

The short version

Creatine increases the amount of stored, immediately available energy in your muscle. More stored energy means more quality reps before output falls off. More quality reps, session after session, means more total work across a training block. More total work is what strength and lean mass are built out of.*

That is the entire chain, and every link in it has been measured.

What creatine is, and where it already lives

Creatine is not a foreign compound. Your body makes roughly a gram of it a day from three amino acids — glycine, arginine, and methionine — and you eat roughly another gram or two if you eat meat and fish. Around 95% of what you carry sits in skeletal muscle.

You also lose some every day. Creatine degrades to creatinine at a steady rate and leaves in urine, which is why intake matters: the tank drains whether or not you train.

Here is the part that makes supplementation worth doing. The tank is not full by default. An ordinary omnivorous diet leaves most people's muscle creatine stores somewhere below their ceiling, and supplementation raises them — research consistently reports increases on the order of 10% to 40%, with the size of the jump depending on how low you started.

More work across the block

This is the one that matters and the one nobody leads with. A few extra quality reps per session is unremarkable on Tuesday. Creatine has been studied in controlled human trials for decades, in hundreds of studies, across trained and untrained populations, across ages, across sports. The International Society of Sports Nutrition maintains a formal position stand on it, and its conclusion is unambiguous: creatine monohydrate is the most effective nutritional supplement available for increasing high-intensity exercise capacity and lean body mass during training. It also sits in the very small group of supplements that major sport-science bodies accept as having good evidence for a performance effect at all. Most of the shelf is not in that group. Most of the shelf is not close to that group.

The safety record

Creatine has been examined in long-term studies running years, in healthy people, at maintenance doses, and no consistent pattern of adverse effects has emerged. The kidney concern that circulates online has been looked for specifically and repeatedly in people with healthy kidney function, and it has not been found.

One thing worth knowing, because it causes real confusion at annual bloodwork: creatine supplementation can raise serum creatinine. Creatinine is the breakdown product of creatine, and if you are carrying more creatine, you produce more creatinine. Serum creatinine is also the number used to estimate kidney function, so a normal, expected consequence of supplementation can look like a red flag on a lab report. If you take creatine, tell your doctor before they interpret that panel. This is not a reason to avoid creatine; it is a reason to mention it.

Who responds most, and who responds least

The size of the response depends on where you start, and this is genuinely useful to know before you judge it on yourself.

If you eat little or no meat, expect a larger response. Dietary creatine comes almost entirely from meat and fish. Vegetarians and vegans typically carry lower baseline muscle creatine and show the biggest increases on supplementation.

If you eat a lot of red meat, expect a smaller one. Some people are already close to their storage ceiling. They are sometimes called non-responders, which is misleading — they responded already, at dinner. There is less room left to fill.

Everyone else is somewhere in between. This is also why judging creatine on a single session is pointless. Stores fill over days to weeks, and the effect you are looking for shows up in accumulated training volume, not in how one Tuesday felt.

What creatine is not

Short list, stated plainly, because clarity here is worth more than enthusiasm.

It is not a stimulant. There is no jolt, no acute effect to chase, nothing you will feel in the thirty minutes after a scoop.

It is not a fat burner and it is not a hormone.

It is not a substitute for training, food, or sleep. It widens the ceiling on the work you can complete. You still have to complete it.

And it does not need to be cycled. There is no evidence that continuous use downregulates anything or that periodic breaks improve the response. Stopping simply lets your stores drift back down over the following weeks.
